User Guide
Follow the main process for using this system: register, log in, submit a request, and use the QR code for your logged book.
1. Register your account
Open register.html and complete the registration form with your personal details.
- Enter your ID Number, name, email, and contact details.
- Upload your selfie as required by the system.
- Submit the form and wait for account activation.
2. Log in
Go to login.html and sign in with your registered credentials.
- Use your ID Number and password.
- Complete any required 2FA or verification steps.
- Access your dashboard after successful login.
3. Submit a medical request
After login, go to medical-request.html or the request section in your dashboard.
- Select the medical tests or certificate type you need.
- Upload required attachments such as test results or supporting documents.
- Review and submit the request for processing.
4. Use the QR code for your logged book
Once your request is approved and issued, use the QR code feature to access the logged book entry.
- Open your issued medical certificate or logged book page.
- Scan the QR code with your phone or verify the code on screen.
- The QR code confirms your request status and logged book record.
