The ISPSC-SM Clinic Records System is used to register students and employees, maintain their clinic visit history, and support the delivery of basic health services within the campus. The system is not a replacement for hospital-level care and does not provide emergency medical treatment.

• Identification details (ID number, name, department, role, date of birth, sex/gender)
• Contact information (mobile number, email address, emergency contact details)
• Basic health and lifestyle information (medical history, family history, allergies, medications, lifestyle habits)
• Clinic visit records and requests (e.g., medical certificate requests, consultations, recommendations)
• Profile photo or clinic selfie used for correct patient identification

Your information is processed only for legitimate clinic and school-related purposes, including:

• Creating and maintaining your electronic clinic record
• Supporting consultations, clearances, and health-related certifications
• Assessing trends and improving clinic services using aggregated, de‑identified data
